Author Info Form

So, this is what I think we need to have authors fill out as soon as we know they're interested in setting up a tour with us. We would include the sign-up on the tour page for them to get started. This way too if we both have access to the google doc either of us can work on it in case you get backlogged. I can take what you can't and run with them, but I won't unless you're okay with me doing that. I think we could handle more tours this way because then there won't be as much going back and forth too, which takes time. We'll still build relationships with authors as we work out the themes and guest posts and such.

Here's what I have. Add too, take away, adjust, re-word, whatever. :)
